We are seeking a dedicated Registered Nurse (RN) with a strong background in home health and OASIS documentation for a 13-week, full-time contract assignment. Schedule:

13-week full-time contract Monday through Friday On-call duties may be required Structured daily visit expectations

Key Responsibilities:

Deliver in-home nursing care and skilled interventions to patients Complete thorough patient assessments and timely, accurate OASIS documentation Work closely with physicians and other healthcare providers to tailor individualized care plans Provide wound care, manage medications, educate patients, and monitor chronic conditions Communicate any changes or updates in patient conditions to the full clinical team

Qualifications:

Current, active RN license At least one year of recent home health RN experience Required proficiency in OASIS documentation Familiarity with HomeCare HomeBase preferred Capable of managing a consistent, full-time schedule including daily patient visits Must have reliable transportation and a valid drivers license
