2988843748-2 Apply for this job now Job Description Aveanna Healthcare is seeking an In Home Healthcare RN to deliver compassionate, skilled nursing care to patients in their homes. In this role, you will perform comprehensive assessments, administer medications and treatments, and develop individualized care plans in collaboration with physicians and an interdisciplinary team. You will educate patients and families on disease management, safety, and self-care while documenting care in an electronic medical record. Responsibilities Provide skilled nursing care to patients in their homes, including assessments, treatments, and medication administration Develop, implement, and update individualized care plans in coordination with physicians and interdisciplinary team Monitor patient conditions, document clinical findings, and report changes promptly to providers and supervisors Educate patients and families on disease management, medications, and home care techniques Coordinate care and communicate regularly with caregivers, therapists, and other healthcare professionals Maintain accurate, timely electronic medical records and comply with regulatory and agency standards Ensure infection control and patient safety practices are followed in the home setting Participate in ongoing training, competency reviews, and quality improvement initiatives Support patient and family emotional needs with compassion and professionalism Travel between patient homes within assigned territory and manage daily visit schedule Required Skills Patient assessment IV therapy and venipuncture Medication administration Wound care management Care planning and care coordination Electronic Medical Records (EMR) documentation Home health regulations and compliance Infection control practices Patient and family education Time and caseload management
