Duties Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews using advanced EMR (Electronic Medical Records) systems such as CradleMRX. Help manage Patient's medications accurately. May assist with IV infusions and medication management in compliance with HIPAA regulations and aseptic techniques. May perform specialized procedures such as catheterization (including urinary and vascular), tube feeding management, ostomy care. Provide critical care interventions in emergency situations, including airway management, acute pain management, trauma response, and neonatal resuscitation procedures within Level I or Level II trauma center environments. Ensure accurate documentation of medical records, including ICD coding (ICD-9/ICD-10), nursing notes, laboratory results, and discharge planning details using EMR/EHR systems like Meditech or Allscripts. Lead patient education efforts on health coaching topics such as chronic disease management, medication adherence, nutrition counseling, and post-acute care instructions. Maintain infection control standards through proper sterilization procedures and aseptic techniques during all patient interactions. Assist with specimen collection/processing for laboratory testing while adhering to laboratory safety protocols. Experience Proven experience working as a Registered Nurse in home health will be beneficial Demonstrated proficiency with EMR/EHR systems platforms for medical documentation and order entry. Prior involvement in case management programs focusing on discharge planning and chronic condition management enhances your suitability for this role. Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) is required; additional certifications like Pediatric Advanced Life Support (PALS) or Trauma Nursing Core Course (TNCC) are a plus.
