The RN case manager will coordinate and assume responsibility for the coordination of patient care with the interdisciplinary team including physicians, patients/family and referring agency. The ideal candidate should have passion for hospice. Our goal is to make sure that our patients and caregivers receive exceptional care and support. The candidate should be compassionate, caring, responsive, dependable and dedicated.

Duties and Responsibilities:

• Ensure each assigned patient's plan of care for nursing cases is established
• Responsible for the quality of patient care provided.
• Ensure that all aspects of patient care are timely and appropriately delivered.
• Coordinate interdisciplinary and inter-agency care as appropriate.

Requirements:

• Clear understanding of federal and state regulations regarding hospice services
• Ability to function independently with minimal supervision.
• Outstanding attention to details.
• Ability to work in a fully automated, computerized environment
• Possess good interpersonal skills
• Possess a sound foundation of nursing skills. Education and Experience
• Current State of Texas Register Nursing License
• Valid State Driver's License and current auto liability insurance
• Computer Proficiency
• Possess strong organizational skills and interpersonal skills.
• At least one year experience with a hospice agency.
• Knowledge of Kinnser/Wellsky is preferred.
