Schedule:

Full-time hours, Monday through Friday On-call responsibilities may be included 13-week assignment, with the possibility of extension

Key Responsibilities:

Deliver in-home nursing care, provide skilled interventions, and ensure accurate documentation using OASIS Complete patient assessments and develop individualized care plans in collaboration with physicians and home health teams Perform wound care, handle medication management, offer patient and caregiver education, and monitor ongoing health conditions Communicate any significant patient updates to the broader clinical team to ensure seamless care

Qualifications:

Active RN license, in good standing At least 1 year of recent RN experience in home health Strong OASIS documentation skills are required Familiarity with HomeCare HomeBase (preferred) Dependable with structured full-time visit schedules Valid drivers license and reliable transportation
