RN needed for home health visits in Pearland and surrounding areas on a full time basis .

Preferred Experience:

Kinnser/Oasis experience preferred with at least one year in home health and strong clinical skills. SOCs, recerts, D/C's and SNVs for mostly ortho and some medically acute patients. Strong analytical/critical thinking skills needed and QA experience a plus. Fast-paced environment which makes the day fly by. Strong, collaborative nursing team that really cares for one another and will make your job both rewarding and FUN!

Work Hours:

30 Units/week Job functions Include but not limited to:

• Admitting/Recertifying/Discharging patients
• Provides nursing care and teaching services to patients and their caregivers
• Evaluates, coordinates and treat patients using the most current technology and practices
• Communicate with the case manager/Clinical manager/DON to decide the initial plan of care based on initial patient assessment and physician's orders
• Be provided with a consistent case load
• Participate in clinical outcomes monitoring, follow up and agency performance improvement initiatives
• Spend more time working with patients and making a significant difference in their lives
• Manage and educate Home Health Aides and LVN's performance in implementing nursing services
• Gain quality experience in working with a multi-disciplinary team and personal patient interaction
• Provide coordination of care between multiple disciplines in the home We are Joint Commission certified and have been in business for 15+ years (this is a big deal in home health and not easy to achieve).
