Flexible shifts available Monday through Sunday Self-scheduling to fit your availability Minimum weekly or bi-weekly hourly commitments Responsibilities include: Performing comprehensive home health risk assessments for Medicare Advantage plan members Evaluating prior and current medical history, including review of medical conditions and medication usage Educating patients on chronic and acute medical conditions, medication regimens, and preventive health strategies Identifying and addressing gaps in patient care, providing actionable recommendations for follow-up Conducting various evaluation types, from annual assessments to post-discharge visits No requirement for creating ongoing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Texas Graduate of an accredited MSN nursing program Board Certification as an NP through AANP or ANCC Current CPR (BLS) certification At least six months of recent clinical NP experience preferred Experience in home health or acute care settings is advantageous In this role, youll experience the flexibility to set your own schedule while contributing to improved health outcomes in diverse communities throughout Williamson County. Compensation for each completed evaluation ranges from $80 to $90, with additional reimbursement for mileage and administrative time.
