Core Responsibilities:

Complete annual home health risk assessments for Medicare Advantage health plan members in their residences Obtain and review prior and current medical history, assess ongoing conditions, and reconcile medications Educate patients on their medical conditions, management, and prescribed medications Identify care gaps and make informed recommendations to support better health outcomes No responsibility for developing treatment plans or prescribing medications

Desired Qualifications:

Licensed Nurse Practitioner in Texas Current BLS certification Board certified by AANP or ANCC Minimum of 6 months of Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited program Compensation for this per diem position is $80-$90 per evaluation, with additional stipends for mileage and administrative time.
