Job Summary We are seeking a dedicated and compassionate Registered Nurse for our Home Health division. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background, excellent communication skills, and the ability to work independently while collaborating with a multidisciplinary team. Duties Conduct comprehensive assessments of patients' health status in their home environment. Develop and implement individualized care plans based on patient needs and preferences. Administer medications and treatments as prescribed, ensuring adherence to aseptic techniques. Monitor vital signs and manage ventilatory support for patients requiring respiratory care. Provide triage services for patients experiencing acute health issues or complications. Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care. Educate patients and families on health management, disease prevention, and home care techniques. Document all patient interactions accurately in compliance with regulatory standards. Skills Proficiency in laboratory procedures and understanding of clinical diagnostics. Experience in Pediatric Intensive Care Unit (PICU) or trauma medicine is preferred. Strong knowledge of home community care practices and medical office operations. Ability to perform triage effectively in various healthcare settings. Excellent interpersonal skills to build rapport with patients and families. Strong organizational skills to manage multiple patient cases efficiently.
