Each visit will typically last around 45 minutes and may be an annual health assessment or a post-discharge check-in for Medicare Advantage plan members.

Key Responsibilities:

Perform in-home annual risk assessments for Medicare Advantage members Review and evaluate patients medical histories, existing conditions, and medication usage Educate patients regarding their health conditions and proper medication use Identify care gaps and offer actionable recommendations No treatment planning or medication prescribing required

Qualifications:

Active Nurse Practitioner license in Texas Current BLS/CPR certification Board Certification via AANP or ANCC At least 6 months of recent NP experience preferred Master of Science in Nursing (MSN) from an accredited institution Compensation includes $80-$90 per evaluation, along with a mileage stipend and support for administrative time.
