Coverage Area:
- Up to 60 miles from the Houston office +
- Contract Length:
- 3-Month Contract, with possibility of extension +
- Schedule:
- Monday-Friday, 8-hour shifts between 7:00 AM and 6:00 PM +
- Pay Rate:
- $53.00 - $55.90 per hour, plus mileage reimbursement between patient visits +
- On-Call Requirement:
- Required during weekdays and weekends +
- Weekend Requirement:
- Required as part of the on-call rotation
- Responsibilities
- + Manage a caseload of hospice patients in home and community settings + Perform hospice admissions, routine patient visits, and end-of-life visits + Complete comprehensive patient assessments and documentation + Coordinate care with physicians, caregivers, and interdisciplinary team members + Monitor patient conditions and communicate changes to the care team + Provide education and support to patients and their families + Deliver compassionate, high-quality hospice care •Requirements•+ Active RN license + Associate degree in Nursing or higher + Recent hospice experience + Recent HCHB (Homecare Homebase) experience + Ability to manage patient visits independently + Reliable transportation for local travel + Strong assessment and documentation skills •Preferred Skills•+ Strong organizational skills + Team-oriented mindset + Excellent communication skills + Ability to manage multiple priorities in a fast-paced environment •Additional Information•+ •
