Genus Patris Hospice has an immediate opening for a Case manager/Admitting Nurse (RN). She/he provides intermittent skilled nursing services, communicates the patient's progress with other disciplines and directs, supervises and instruct non-professional hospice aide staff in the provision of personal care to the patient. Hospice Nurses are registered nurses who provide care for terminally ill patients. They work with patients, their families and other healthcare professionals during the final stages of life.
Responsibilities:
Assessing patients' social and medical history to develop plans of care that will fit and meet their needs. Working with physicians, nurses, and other medical professionals to coordinate care and treatment plans for each patient Coordinating and arranging for services such as home health aides, nursing care, and hospice care Maintaining records of patient care plans, physician orders, medication administration, and other information about a patient's condition Coordinating care with other medical providers , chaplain and social services agencies in order to meet patients' needs Taking care of Hospice compliance with nurses and staff documentation.
Qualifications:
A. Is currently licensed in the State of Texast in accordance with the Board of Nursing Examiners rules for Nurse Licensure Compact ( NLC) and B. One (1) year experience as professional nurse especially hospice preferred C. Ability to work in the field setting and exhibited ability to make sound nursing judgement. D. Ability to assess patient needs and formulate individualized patient care plans with interdisciplinary team to meet patient needs. E. Effective communication skills F. Must have and maintain an automobile to be used for work. G. Bilingual a plus
