This per diem opportunity is ideal for Nurse Practitioners seeking a flexible schedule while serving communities throughout Williamson County. The role focuses on providing comprehensive annual health assessments and post-discharge visits, working with patients in Mount Pleasant, Winfield, Talco, and surrounding areas.

Key Responsibilities:

Complete in-home annual risk assessments for Medicare Advantage plan members Review and document prior/current medical history, conditions, and medications Educate patients on medical issues and prescribed medications to foster understanding and compliance Identify gaps in care and recommend resolutions to enhance patient outcomes Focus solely on assessments (no treatment planning or prescribing required)

Qualifications:

Active Nurse Practitioner license in Texas Current CPR certification (BLS required) Board Certified through AANP or

ANCC MSN

from an accredited nursing program At least 6 months of recent NP clinical experience preferred
