Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in their residences throughout Wichita Falls and surrounding areas, including Burkburnett, Electra, and Kamay. Review and evaluate patients prior and current medical history, chronic and acute medical conditions, and medication usage. Educate patients on their health conditions and prescribed medications. Identify gaps in care, making recommendations to ensure continuity and quality of care. Visits focus on assessment and education; no treatment plans or prescriptions are required.

Qualifications:

Active Nurse Practitioner license in Texas Board Certification through AANP or ANCC Graduate of an accredited MSN program Current CPR/BLS certification At least 6 months of Nurse Practitioner experience preferred This role offers compensation of $80-$90 per evaluation, plus a mileage stipend and consideration for administrative time.
