Join the Thatcher Brook Rehabilitation & Care Team Thatcher Brook Rehabilitation & Care is seeking a compassionate and dedicated RN or LPN to join our team. As an nurse, you will play a key role in providing high-quality, resident-centered care in our skilled nursing and rehabilitation community. You will work closely with residents, families, physicians, therapists, and fellow caregivers to develop and implement individualized care plans that promote healing, comfort, independence, and quality of life. Responsibilities Assess residents' physical, emotional, and psychosocial needs and develop individualized nursing care plans. Administer medications, treatments, and physician-ordered therapies safely and accurately. Monitor residents' conditions, recognize changes in health status, and communicate concerns promptly to the healthcare team. Coordinate care with physicians, rehabilitation therapists, social services, and other interdisciplinary team members to ensure optimal resident outcomes. Provide skilled nursing care including wound care, IV therapy (as permitted), catheter care, enteral feeding, and other clinical procedures within scope of practice. Maintain accurate and timely documentation in the electronic health record (EHR). Educate residents and families regarding diagnoses, medications, treatments, and discharge planning. Supervise and support Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs) while fostering a positive team environment. Respond appropriately to emergencies and changes in resident condition. Ensure compliance with state and federal regulations, infection prevention standards, and facility policies. Promote resident dignity, independence, and quality of life through compassionate, individualized care. Qualifications Current Registered Nurse (RN) license in good standing. Graduate of an accredited nursing program. Previous experience in skilled nursing, rehabilitation, long-term care, subacute care, or geriatric nursing is preferred but not required. Strong clinical assessment and critical thinking skills. Experience with electronic medical records (EMR/EHR) systems preferred. Excellent communication, organizational, and interpersonal skills. Ability to work collaboratively with interdisciplinary teams and build positive relationships with residents and families. Current CPR/BLS certification or the ability to obtain upon hire.
