Job Description
Registered Nurse (RN) Full-Time | Home Health and Hospice | Layton, Utah Position Summary CNS is seeking a compassionate, full-time Registered Nurse to support Home Health and Hospice patients in the Layton area and surrounding communities. In this role, you'll help patients recover and stay independent at home, and provide comfort and dignity to hospice patients and their families when it matters most. This role offers meaningful 1:1 care, greater autonomy, and the opportunity to work with a mix of adult and some pediatric patients.
Compensation Hourly:
Compensation starting at $34.04 more based on experience and education.
Mileage Reimbursement:
Mileage reimbursement is provided at $0.60 per mile for qualified travel.

Schedule:
Monday - Friday, 8am to 5pm
On-Call:
This position requires on-call availability as needed and holiday rotation. 27 total benefit days first year! • Key Responsibilities Provide excellent care within the scope of your practice and licensure Perform comprehensive nursing assessments and develop individualized plans of care for Home Health and Hospice patients Complete skilled nursing visits, admissions, recertifications, resumptions of care, and discharges, including accurate OASIS documentation for Home Health patients Provide hospice nursing care focused on pain and symptom management, comfort, and quality of life Support patients and families through end-of-life decisions, goals-of-care conversations, and difficult health transitions Administer medications, IV therapy, injections, and treatments as ordered, monitoring patient response Provide wound care, catheter care, ostomy care, and other skilled nursing procedures Educate patients, families, and caregivers on disease management, medications, safety, and what to expect Coordinate care with physicians, advanced practitioners, social workers, chaplains, therapists, CNAs, and the interdisciplinary team (IDT) Recognize changes in patient condition and communicate promptly with providers and the care team Maintain timely, accurate documentation in compliance with Medicare, Medicaid, CMS, State of Utah, and CNS standards Adhere to infection control, safety, and HIPAA requirements Care for pediatric, adult, and geriatric patients as assigned Qualifications Education & Licensure Current, active Utah RN license (or multistate compact license) in good standing Graduate of an accredited nursing program (ADN required; BSN preferred) Experience & Knowledge Minimum of 1 year of RN experience required Home Health and/or Hospice experience preferred OASIS experience preferred (training provided) Understanding of Medicare and Medicaid home health and hospice guidelines preferred Knowledge of pain and symptom management, wound care, and chronic disease management Skills & Abilities Compassionate, patient-centered approach to clinical care Strong communication and interpersonal skills with the ability to build rapport with patients and families Ability to work independently with minimal supervision in diverse home settings Strong organizational and time management skills with the ability to manage a field-based caseload Strong critical thinking and assessment skills with attention to detail in documentation Cultural competency and sensitivity to diverse populations Proficiency with electronic medical records and documentation systems Ability to handle emotionally challenging situations with professionalism and empathy Other Requirements Reliable transportation and willingness to travel to patients in the surrounding communities your location supports Valid driver's license and current auto insurance Ability to manage a field-based caseload and flexible schedule Comfortable working in diverse home environments Ability to pass a criminal background check and drug screening Current CPR certification (or obtained within 30 days of hire) Physical Requirements Ability to lift, transfer, and reposition patients (up to 50 lbs frequently, more with assistance) Ability to drive throughout the area served by your assigned CNS office Ability to sit, stand, walk, bend, kneel, and squat for extended periods Ability to navigate stairs and various home environments Physical stamina to provide hands-on patient care throughout the day This job summary outlines the general nature and level of responsibilities and is not an exhaustive list of all duties or skills required.
