Assist with the admission process by providing timely assessments to determine Assisted Living eligibility. Coordinate with the Health & Wellness team to determine care needs of individual residents through routine and change of condition assessments. Provide training and delegation to Med Techs. Assure resident medication assistance meets regulatory requirements. Participate in Clinical Team meetings and Care Conferences with residents, families and Outside Agency partners. Follow up on all resident incidents- reviewing documentation and giving instructions to staff and residents while addressing any preventative measures or changes to Care Plan.
