Job Summary:
Provides medical and professional consultation on patient care related issues. Acts as professional advisor and consultant to Caregiver Support Network's (CSN) overall program through the professional advisory committee. Manage the overall services for the client/family. Responsible to make sure that the client's/family's needs are being met either by you or the other personnel you have assigned. It is the Care Manager's responsibility to design the Plan of Care and do whatever it takes to assure the appropriate delivery of services according to the Plan of Care. This requires an intimate involvement with the interdisciplinary team and the client/family. We offer a flexible schedule and a great team work environment. $500 sign on bonus for part time employees.Job Type:
