We are currently seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide exceptional healthcare services to patients in their homes. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for delivering personalized care to individuals in need. This is a part-time position. We make every effort to accommodate your personal schedule.
Responsibilities:
Conduct comprehensive assessments of patients' health status, including physical, emotional, and social factors. Develop and implement individualized care plans based on assessment findings and in collaboration with the patient, their family, and other healthcare team members. Provide skilled nursing care, including wound care, medication management, infusion therapy, and other treatments as prescribed by the physician. Monitor patients' progress and response to treatment, and make necessary adjustments to the care plan. Educate patients and their families about their medical conditions, treatment options, and self-care techniques to promote optimal health and independence. Coordinate care with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to ensure continuity of care and comprehensive support for patients. Document all patient assessments, interventions, and outcomes accurately and in a timely manner, adhering to regulatory and organizational standards.
Qualifications:
Valid Registered Nurse (RN) license in the state of Utah. BLS certification required; ACLS certification preferred. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and interdisciplinary team members. Knowledge of Medicare regulations and documentation requirements for home health services. Ability to work independently and prioritize tasks effectively in a fast-paced environment. Reliable transportation and willingness to travel to patients' homes. Valid driver's license and proof of insurance.
