About Us Mindful Mobile Medicine, a sister company of Refined, is a mobile healthcare practice dedicated to bringing personalized, high-quality medical care directly to older adults in assisted living, independent living, and memory care settings. Our approach is rooted in compassion, collaboration, and a deep commitment to improving the lives of aging individuals through community-based care. Position Overview We are seeking a Geriatric Nurse Practitioner (APRN) with a specialization in geriatrics to join our expanding team. This role is ideal for an experienced provider passionate about elder care and looking to deliver patient-centered, in-home medical services. You will work in close collaboration with our Licensed Clinical Social Workers (LCSWs) and facility staff to support holistic, integrated care. Responsibilities Provide comprehensive, in-person medical care to older adults in their homes, assisted living, and independent living communities Perform geriatric assessments, diagnose and manage chronic and acute conditions common to aging populations Coordinate care with behavioral health professionals (LCSWs), caregivers, and facility staff Order and interpret diagnostic tests and manage medications appropriately Maintain accurate and timely documentation in accordance with clinical and regulatory standards
