A Plus Healthcare at Home is growing into Park City, and we're looking for a PRN Registered Nurse to help us bring home health care to patients in Park City and the surrounding areas. This is a flexible role that lets you build a schedule around your life. Pay:

negotiable based on experience $0.50/mile reimbursement One on one care in the patient's home Supportive team environment Opportunity to grow into a full-time position as the territory grows What You'll Do Complete initial and ongoing patient assessments, including physical assessment and health history Develop, initiate, and revise the plan of care with the patient, family, and physician Provide skilled nursing care, medication administration, and treatments as ordered, including care for patients recovering from surgery Educate patients and families on their condition, medications, and self-care Serve as case manager for assigned patients, coordinating care with therapy, aides, and community providers Communicate changes in condition to the physician and obtain orders as needed Complete timely, accurate clinical documentation Supervise home health aide care per regulatory requirements Plan for discharge from the start of care We're Looking For Someone Who Holds a current Utah RN license and CPR certification Has strong clinical assessment and critical thinking skills Is comfortable working independently in the home setting Has excellent communication and documentation skills Is reliable and patient focused Has a valid driver's license, reliable vehicle, and auto insurance Home health experience is preferred but not required

Physical Requirements:

Able to walk, stand, reach, bend, and kneel for extended periods, and to lift, position, or transfer patients and carry supplies.
