PRN | On-call Friday 5pm - Monday 8:30am.

Schedule & Coverage Frequency:

PRN, 1 to 2 weekends a month, more if you'd like. Patients scheduled as close to home as possible; occasional travel to surrounding areas as needed.

Position Overview We are seeking a PRN Registered Nurse (RN) to join our team and support our Home Health and Hospice programs in the Price, Utah area. The RN will provide routine visits and admissions while working closely with patients, families, and the care team. Key Responsibilities Perform routine patient visits, assessments, and admissions for home health and hospice. Deliver compassionate, high-quality nursing care in accordance with each patient's individualized plan of care. Communicate effectively with patients, families, and interdisciplinary team members. Document all care provided in compliance with agency standards and regulations. Educate patients and caregivers on care needs, medications, and health management. Maintain professional boundaries while providing empathetic, supportive care. Travel within the greater Utah County region (mileage reimbursement provided). Qualifications Current Registered Nurse (RN) license in the state of Utah. Minimum of 1 year of clinical nursing experience; home health or hospice preferred. Strong communication, organizational, and assessment skills. Ability to work independently and manage time effectively. Valid driver's license, reliable transportation, and proof of insurance.
