3003329341-2 Apply for this job now Job Description Genesis HCC, a leader in skilled nursing and post-acute care, seeks a Wound Care and Skin Health Team Lead, RN to oversee clinical wound programs and champion skin integrity. In this role, you'll perform and guide comprehensive wound assessments, develop evidence-based care plans, and manage complex wound treatments. You will educate and mentor nursing staff, monitor risk and outcomes, and collaborate closely with physicians, therapists, and families. Responsibilities Lead wound care and skin integrity program for post-acute and long-term care patients Perform comprehensive wound and skin assessments and develop individualized care plans Oversee and perform advanced wound treatments and dressing changes per evidence-based standards Educate and mentor nursing staff on wound prevention, staging, documentation, and treatment protocols Monitor skin breakdown risk, implement prevention strategies, and coordinate interdisciplinary interventions Track and report wound care quality metrics, outcomes, and regulatory compliance Collaborate with physicians, therapy, dietitians, and families to optimize healing and comfort Participate in policy development, audits, and continuous improvement initiatives for wound care Required Skills Wound assessment and staging Advanced wound treatment modalities Skin integrity and pressure injury prevention Care planning and clinical documentation Infection prevention and control RN clinical leadership and team coordination Regulatory and quality compliance in long-term care Patient and family education Interdisciplinary care coordination Electronic health record (EHR) documentation
