Current state license as a Registered Nurse; Homecare Homebase Software Experience

• Required; OASIS Experience
• Required; Proof of current CPR certification
• Required; Must have reliable transportation and insurance
• Required; 1-2 years experience as a Registered Nurse in a clinical care setting
• Required; 2+ years Home health experience
• Required; Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program
• Required.

Responsibilities:

Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs. Implement, develop, and evaluate the Plan of Care (POC) to ensure quality and continuity of care. Educate patients and families regarding care, medications, and safety, using appropriate teaching methods. Supervise and provide clinical directions to

HHA/STNA/LVN
