Deliver high-quality patient care in a dynamic field-based setting as a Home Health Registered Nurse (RN). This role provides the unique opportunity to work independently, offering skilled nursing services to patients in their homes and supporting their health and recovery journeys with personalized, compassionate clinical care.

Schedule:

Monday Friday, 8:00 AM 5:00 PM 13-week full-time contract

Caseload:

2532 points per week (typically 35 patient visits per day)

Responsibilities:

Conduct comprehensive OASIS assessments and develop individualized care plans. Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management. Monitor patient progress, promptly identify changes in condition, and communicate with providers and care teams. Educate patients and families about disease management, medications, and preventive care. Complete accurate and timely clinical documentation using Home Care Home Base (HCHB). Collaborate with a multidisciplinary team, including physical and occupational therapists, social workers, and home health aides. Attend case conferences, staff meetings, and participate in ongoing learning activities. Ensure compliance with all regulatory and agency documentation and patient care standards. Travel daily within assigned territory (mileage reimbursement provided).

Qualifications:

Active RN license in good standing for the state of practice. Minimum 1 year recent home health experience (field-based strongly preferred). Proficiency with OASIS assessments and Home Care Home Base (HCHB). Excellent assessment, critical thinking, and patient communication skills. Reliable transportation, valid drivers license, and auto insurance. Ability to organize, prioritize, and manage a flexible home-based schedule. BSN and knowledge of Medicare/home health regulations preferred.
