Up to 50 miles one way Job Summary We are seeking a dependable and experienced Registered Nurse (RN) Contractor to complete in-home nursing assessments for our home care agency. This is a mobile position that requires traveling to clients' homes to perform initial onboarding assessments, routine supervisory visits, and reassessments. This role is ideal for an RN who enjoys working independently, has strong assessment skills, and is committed to delivering high-quality patient care. Responsibilities Complete initial nursing assessments for new home care clients. Conduct routine supervisory visits and reassessments to evaluate clients' health status and ongoing care needs. Review medications, medical history, functional status, and home safety. Develop, update, and document individualized plans of care. Accurately complete and submit all required assessment documentation within agency timeframes. Communicate changes in a client's condition to the appropriate healthcare providers and agency staff. Ensure all visits and documentation comply with agency policies and state regulations. Maintain professionalism and provide compassionate, patient-centered care during every visit. Qualifications Active and unrestricted Registered Nurse (RN) license. Minimum of one year of nursing experience; home care experience is preferred. Excellent assessment, communication, and documentation skills. Ability to work independently with minimal supervision. Reliable transportation, valid driver's license, and current auto insurance. Must be willing and able to travel up to 50 miles to complete scheduled client visits.
