Job Description
The successful candidate will be available to serve as a Telepresenter (Licensed Practical Nurse
- LPN or degreed Medical Assistant
- MA) for the PCPS-Valley Health Telemed Program for the 2026-2027 school year.
This program is currently slated for Luray Middle School, but the candidate may work from any division office or school at any time as a medical professional at the originating site that presents a student-patient to the physician or practitioner at the distant site. The Telepresenter can be a Licensed Practical Nurse (LPN) or a degreed Medical Assistant (MA) who is trained to use technology, such as digital stethoscope, otoscope, examination camera, etc., to facilitate comprehensive exams under provider guidance and will work at the school under the supervision of the school nurse and building principal. The Telepresenter will be required to follow all Valley Health and PCPS policies and regulations. The Telepresenter must be able to lift objects weighing up to approximately 25 pounds frequently, and up to approximately 75 pounds occasionally. This is a one-year, full-time benefit eligible, grant funded position in collaboration with Valley Health.

GENERAL DEFINTION AND CONDITIONS OF WORK

ESSENTIAL FUNTIONS/TYPICAL TASKS
The Telepresenter will work full-time at the top scope of practice to guide and facilitate the Telemed program and visits as well as to improve care access and outcomes with patient care coordination and assistance with population health initiatives. Specifically, the role will include:
Strategizing for, initiating and facilitating the creation of and day to day requirements of a specialty care Telemed program.
Pre-visit planning, assessment, and medication reconciliation for Telemed visits.
Presenting and facilitating Telemed visits with distance providers and then wrapping up the visits with patient education and assisting patients with visit follow-up planning to ensure adherence with established care plan.
Thoroughly document the Telemed visit and ensure that required distant provider documentation is in the electronic health record.
The Telepresenter will be able to perform the following in the area of
Clinical Support:
Review and update medical records ensuring all patient information is properly documented in accordance with practice expectations.
Provide competent assessment, care and treatment appropriate to the age of patients.
Deliver care in a manner that preserves patient autonomy, dignity and rights.
Recognize and report to appropriate personnel if signs of child, elder or domestic abuse are noticed.
Perform on-site tests and procedures inherent to practice setting (i.e. point of care) using appropriate techniques while ensuring that the criteria for billing/compensation has been satisfied (i.e. ABN, ICD and CPT codes).
Correctly prepare and administer medications, injections, and vaccines as ordered by provider. Document appropriate information in the patient's medical record in accordance with practice setting.
Complete routine office clinical functions including but not limited to processing patient phone calls, e-prescribing/medication refills, and processing faxes in an accurate and timely manner.
Accurately and efficiently prepare patient/room/supplies/equipment while adhering to provider preferences.
Provide education to the patient that includes but is not limited to treatment plan, procedures, medications and general health information.
Accurate scheduling of tests, procedures, and consultations with appropriate patient notification. Ensures information is documented in patient's health record.
Accurate scheduling of office appointments.
Ensure that surgical instruments are properly disinfected/sterilized.
Maintain tracking logs pertinent to the medical practice (i.e. medication, glucometer control, critical lab, sample, refrigerator temperature).
Adhere to organizational policies and procedures including those involving HIPAA and safety standards (i.e. biohazard disposal, national patient safety goals, medical record release).

Assist in keeping exam rooms/supply storage areas adequately stocked and cleaned while ensuring supplies are not expired.
Serve as a resource by assisting in orientation and ongoing education of other staff.

Respond to patient and customer complaints in a timely manner. Notifies appropriate staff of resolution.
Utilize resources as necessary to obtain answers to questions encountered while completing job duties.

The Telepresenter is able to exhibit the following as part of
Federal/ State/ Joint Commission Readiness/ Compliance:
Ensuring continual preparedness by participating in activities, education, and successfully demonstrating knowledge in the performance of daily duties.
When requested, actively participates in mock surveys, committees or other activities related to federal/state/Joint Commission regulations.
Provides prompt and accurate data collection and reporting.
Documents or notifies appropriate personnel of concern/safety issues and makes suggestions for improvement.
The Telepresenter will complete other duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES
Decision-making skills are a significant skill of the job, affecting a large segment of the school, students, and teachers. Candidate must possess knowledge of nursing practices and be willing to be a team player who can take direction from supervisors.
EDUCATION AND EXPERIENCE
Must hold a current license as a licensed practical nurse or medical assistant from the Virginia State Board of Nursing. Candidate must hold a current certification in CPR
- American Heart Association or American Red Cross.
SPECIAL REQUIREMENTS
Candidate must possess good moral character.
PHYSICAL DEMANDS/REQUIREMENTS
Duties performed typically in school settings to include: school clinic, offices, conference centers, gymnasium, playground areas, and classrooms. Frequent walking, stooping, standing, lifting, up to approximately 25 pounds and occasional lifting of equipment and/or materials weighing up to 75 pounds occasionally. Other limited physical activities may be required. Frequent contact with the students, school administration, school and division personnel, Valley Health officials and staff, other health agencies, and parents is a requirement. Vocal communication is required for expressing or exchanging ideas by means of the spoken word; hearing is required to perceive information at normal spoken word levels; visual acuity is required for preparing and analyzing written or computer data, determining the accuracy and thoroughness of work. Travel may be required. Candidate must possess the ability to work with special needs students. Frequent contact with parents by phone and in person may be necessary.

The building administrator, in collaboration with the supervising RN, will evaluate performance on the ability and effectiveness in carrying out the above responsibilities.
