Monday Friday, 8:00 AM 5:00

PM Responsibilities:

Perform comprehensive OASIS assessments and develop individualized care plans for each patient. Deliver skilled nursing care, including wound treatment, medication administration, IV therapy, and chronic disease management. Monitor patient progress, promptly report changes in condition, and communicate with care teams and providers. Educate patients and families about disease processes, medications, and preventive health strategies. Accurately and efficiently document all patient interactions in Home Care Home Base (HCHB). Collaborate with other healthcare professionals, including therapists and social workers, to assure consistent and coordinated care. Attend case conferences, team meetings, and participate in ongoing training or continuing education as required. Comply with all agency policies and regulatory standards to promote quality patient care.

Qualifications:

Current RN license in good standing for the state of practice. At least 1 year of recent home health experience (field experience highly preferred). Proficiency in Home Care Home Base (HCHB) and OASIS documentation is required. Valid drivers license, reliable transportation, and active auto insurance are necessary.
