Schedule:

Full-time, 13-week contract Monday Friday, 8:00 AM 5:00 PM Field-based role with daily travel to patient homes within an assigned area

Key Responsibilities:

Complete comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing interventions such as wound care, IV therapy, chronic disease management, and medication administration Monitor patient progress, promptly identify changes in condition, and coordinate with the healthcare team and physicians Educate patients and families on disease management, medication instructions, and preventive care strategies Maintain thorough, timely documentation using Home Care Home Base (HCHB) Participate in interdisciplinary case conferences, ongoing staff meetings, and required continuing education Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides to achieve optimal patient outcomes

Required Qualifications:

Current, active RN license in the state of practice and in good standing At least one year of recent field-based home health nursing experience Proficiency with Home Care Home Base (HCHB) electronic documentation Demonstrated experience with OASIS assessments Strong critical thinking, assessment, and patient communication skills Reliable transportation, valid drivers license, and active auto insurance Ability to function independently and manage a flexible daily schedule Excellent time management and organizational abilities

Preferred Qualifications:
