Job Description
Kaiser Permanente seeks an LMSW Case Manager to deliver compassionate, patient-centered care within our Healthcare & Medical Services department. In this role, you will assess psychosocial needs, develop care plans, coordinate services, and advocate for patients across settings. You will collaborate closely with physicians, nurses, and community providers to reduce barriers to care, support treatment adherence, and improve health outcomes. You'll document according to regulatory standards, track progress, and participate in quality initiatives, all while contributing to a supportive, collaborative, and improvement-focused culture.
