Develop, implement, and evaluate individualized care plans based on patient needs. Administer skilled nursing care such as wound care, IV therapy, catheter care, medication administration, and chronic disease management. Monitor patient progress and communicate changes to the physician and care team. Educate patients and families on disease processes, medication management, and self-care techniques. Physician should be well versed with PleurX Document all visits and interventions accurately and timely in the electronic medical record (EMR). Ensure compliance with all applicable federal, state, and local regulations and company policies. Collaborate with interdisciplinary team members to provide holistic and coordinated care.
