Responsibilities:
Conducts face-to-face encounters for patients in their third or subsequent benefit period, documenting and sharing findings with certifying physician promptly. Conducts well-patient checks. Conducts symptom management visits. Provides physician support. Coordinates and collaborates with IDT members, community physicians, facility staff, and others as needed. Participates in developing Plan of Care for assigned patients. Assists in enhancing care strategies with Hospice Clinical Manager and Medical Director. Acts as a resource for Hospice staff to ensure continuity and improve patient care quality. Attends IDT meetings. Provides backup support to Hospice Medical Director, covering medical needs of hospice patients as required The Difference You'll Make This is more than a clinical role. Requirements:
Graduate of an approved Nurse Practitioner curriculum Active APRN or NP license required Minimum of two years clinical nursing experience Valid driver's license Ready to Make a Difference?
