May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

Job Description
Department Description:
The Community Hospice team delivers care to patients in the community, providing a wide range of skilled nursing care and support. We are an interdisciplinary team all collaborating together to support patients to remain in the setting of their choice, their home. The RN will provide a full range of skilled nursing care to patients in a home care setting with a focus on patient education and palliative care to meet the physical and emotional needs of patients, while educating and providing support to families. This is a per diem position. We strongly support educating new team members with an individualized and comprehensive orientation. To aid in a successful onboarding with maximum support, candidates must be prepared to commit to full time hours for the first 6-8 weeks. After orientation, we are looking for a minimum commitment of 20 hours a month, and availability on weekends and overnights. Minimum Requirements Associate's degree in nursing or higher. Minimum of one or more years of directly related home or community hospice nursing experience OR, two or more years of related home or public health nursing experience OR, three or more years of general nursing experience, including at least one year of directly related home/public health or hospice nursing experience Certification in hospice and palliative care preferred.On-call:

Requirements:
