RN Case Managers at Facility, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.

Minimum Qualifications:

Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license. Graduate of an accredited school of nursing. Medical/Surgical/Oncology nursing and/or hospice care experience preferred.
