None Position Summary:

We are seeking an experienced Registered Nurse (RN) Case Manager for a temporary assignment in Windsor, Vermont. The RN Case Manager will coordinate and manage interdisciplinary care for patients across the healthcare continuum, including hospital, home, rehabilitation, skilled nursing, and outpatient settings.

Key Responsibilities:

Coordinate interdisciplinary care for defined patient populations across the healthcare continuum. Conduct comprehensive patient and family assessments, including clinical, psychological, social, and environmental needs. Identify high-risk patients requiring ongoing care coordination. Develop, implement, and monitor individualized treatment and care plans. Coordinate transitions of care between hospitals, clinics, homes, skilled nursing facilities, rehabilitation centers, and other care settings. Advocate for patients and families while identifying and removing barriers to accessing healthcare services. Coordinate benefits and continuing-care services with insurance plans and other healthcare organizations.

Required Qualifications:

Minimum 3 years of clinical nursing experience Recent acute-care hospital experience required BSN required Previous travel nursing experience preferred Utilization Review experience is a plus

Pay:

$2,600.00 - $2,800.00 per week
