Pine Ridge Post Acute Make a Difference From the Very Beginning At Pine Ridge Post Acute, the admissions process is more than paperwork — it's the beginning of a resident's next chapter. We're looking for an Admissions Nurse who is compassionate, organized, clinically confident, and enjoys working with people to help make transitions into skilled nursing as smooth and comfortable as possible. Part-Time $30-$53 per hour In this role, you'll be an important connection between hospitals, physicians, residents, families, and our interdisciplinary team. You'll use your nursing expertise to help determine whether we can safely meet a prospective resident's needs while making sure every admission is thorough, accurate, and compliant. What You'll Do Review clinical referrals and medical information to determine admission appropriateness. Complete comprehensive clinical reviews, including diagnoses, medications, treatments, therapy needs, labs, infection status, behavioral concerns, and functional abilities. Communicate with hospitals, discharge planners, physicians, families, pharmacies, transportation providers, and other referral partners. Collaborate closely with Nursing, Administration, Therapy, Social Services, Dietary, and other members of the interdisciplinary team. Review physician orders, medication lists, therapy recommendations, and discharge documentation before admission. Coordinate with nursing and ancillary departments to help ensure rooms and resources are ready for new residents. Complete or oversee nursing admission assessments and required documentation. Ensure accurate medication reconciliation, physician orders, consents, care plans, and electronic medical record documentation. Initiate admission care plans and communicate resident needs to the care team. Coordinate infection prevention and isolation precautions when needed. Maintain accurate admission logs, referral tracking, and required reports. Serve as a clinical resource for referral sources regarding services available at Pine Ridge. Support quality improvement efforts related to admissions, transitions of care, and reducing rehospitalizations. Help ensure admission practices remain compliant with federal regulations, Washington State requirements, and facility policies. Protect resident privacy and maintain HIPAA compliance. What We're Looking For Current Washington State license in good standing. Minimum 2 years of nursing experience . Graduate of an accredited nursing program. Current CPR/BLS certification . Strong clinical assessment, critical-thinking, and decision-making skills. Excellent communication and organizational skills. Ability to manage multiple referrals and priorities in a fast-paced environment. Basic computer skills and experience with electronic medical records; PointClickCare experience is preferred . Knowledge of Medicare, Medicaid, managed care, and post-acute care regulations is preferred. Experience in skilled nursing, rehabilitation, hospital case management, admissions, utilization review, or discharge planning is a plus.
