This is a non-prescribing, field-based role focused on preventive care, patient education, care gap closure, and collaboration with primary care providers. Key Responsibilities Conduct comprehensive in-home health assessments Review medical history and medications Perform physical exams, screenings, and point-of-care testing Identify and document diagnoses Communicate findings to PCPs Educate patients on health conditions and treatment plans Address social determinants of health and connect patients with resources Recognize and respond to urgent clinical situations

Required Qualifications NP:

ANCC or AANP certification (Family, Adult, Geriatric, or AGPCNP)

PA:

Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
