Key Responsibilities:

Complete annual home health risk assessments in patient residences Assess prior and ongoing medical history, current conditions, and medications Educate patients about their health conditions and medication management Recognize any gaps in care and propose solutions

Qualifications:

Active Nurse Practitioner licensure in Washington Current CPR certification (BLS required) Board certified via AANP or ANCC At least 6 months of recent nurse practitioner experience (preferred) Master of Science in Nursing (MSN) from an accredited institution In this rewarding position, youll earn between $80 and $90 per completed evaluation, with a mileage stipend and compensation for administrative tasks included.
