Key Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage members Review and evaluate medical history, chronic conditions, and medication lists during each visit Provide clear education to patients on their health conditions and prescribed medications Identify care gaps and suggest practical, actionable recommendations to help close them All visits are assessment-only with no requirements for developing treatment plans or prescribing medications

Qualifications:

Active and unrestricted Nurse Practitioner license in Washington State Current Basic Life Support (BLS/CPR) certification Board Certification through the

AANP or ANCC Preferred:
