Responsibilities:

Conduct in-home risk assessments for Medicare Advantage members Review and evaluate medical histories, current health conditions, and medication regimens Provide patient education regarding medical conditions, treatment adherence, and medications Identify care gaps and recommend appropriate next steps to optimize health outcomes Visits average approximately 45 minutes; no medication prescribing or treatment plan development required

Qualifications:

Active Nurse Practitioner license in the state of Washington Board Certification through AANP or ANCC Current CPR (BLS) certification Masters degree in Nursing (MSN) from an accredited school At least 6 months of recent NP experience (preferred)
