Key Responsibilities:

Conduct annual and post-discharge home health risk assessments for Medicare Advantage members. Assess patients prior and current medical histories, conditions, and medication profiles. Deliver personalized education regarding medical conditions and medication regimens. Identify health gaps and provide actionable recommendations for optimal outcomes. Focus solely on assessment and educationno need to develop treatment plans or prescribe medications.

Qualifications:

Active Nurse Practitioner License in Wisconsin Board Certification via AANP or ANCC Current CPR certification (BLS) Graduation from an accredited MSN program Minimum of 6 months current experience as a Nurse Practitioner preferred Youll be compensated with $80$90 per evaluation, plus a stipend for mileage and administrative time, reflecting your expertise and commitment to community health.
