A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.
Position Summary We are seeking an experienced Nurse Consultant to provide clinical leadership and support to skilled nursing facilities throughout Wisconsin. This role partners with Administrators, Directors of Nursing, and clinical teams to improve resident care, maintain regulatory compliance, strengthen nursing operations, and support quality improvement initiatives. Our client is an established and growing long-term care organization that acquired four senior care communities in 2018 and 2019, including two Skilled Nursing Facilities (SNFs) and two Assisted Living Facilities (ALFs).

The role supports facilities in both Milwaukee and Beloit, Wisconsin . A corporate office in Chicago is available as a home base if preferred. Travel between Chicago and the facilities is manageable, with Milwaukee approximately 1 hour and 10 minutes from Chicago, Beloit approximately 1 hour and 25 minutes from Chicago, and just over an hour between the two Wisconsin campuses. Key Responsibilities Provide clinical guidance and support to Directors of Nursing and nursing leadership teams. Conduct routine facility visits, clinical audits, and assessments to identify opportunities for improvement. Monitor resident care, quality outcomes, documentation, staffing, and regulatory compliance. Assist facilities with survey preparation, survey response, plans of correction, and ongoing compliance. Review high-risk clinical areas, including falls, wounds, infections, medication management, hospitalizations, and changes in condition. Develop and monitor corrective action plans to address clinical and regulatory concerns. Mentor nursing leaders through coaching, education, and leadership development. Support Quality Assurance and Performance Improvement (QAPI) initiatives. Collaborate with interdisciplinary teams to promote high-quality resident care. Provide onsite support during surveys, leadership transitions, and other critical clinical situations. Communicate facility performance, risks, and recommendations to regional leadership. Qualifications Current Wisconsin Registered Nurse (RN) license or eligibility to obtain licensure. Minimum of five years of nursing leadership experience in skilled nursing or long-term care. Previous Director of Nursing experience preferred. Strong knowledge of CMS regulations, survey processes, quality measures, and long-term care standards. Experience with clinical audits, regulatory compliance, and performance improvement. Excellent leadership, communication, organizational, and problem-solving skills. Valid driver's license and ability to travel regularly throughout Wisconsin. Preferred Qualifications Regional or multi-facility clinical leadership experience. Experience supporting facilities during state and federal surveys. Experience with electronic health records (EHR) and clinical risk management systems. Travel Regular travel to assigned facilities throughout Wisconsin is required, with flexibility to provide support during surveys, emergencies, and other critical clinical situations.
Pay:
$100,000.00 - $130,000.00 per year
