A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.
Job Summary We are seeking a compassionate and dedicated Registered Nurse to join our Assisted Living and Memory Care facility team. The ideal candidate will provide high-quality patient care in a medium-paced environment. This role requires strong clinical skills, effective communication, and the ability to work collaboratively with multiple people. The primary goal is to provide on-site supervision of the residents at Anita's Gardens, which includes, but is not limited to; overall responsibility for management of health, communication with physicians, residents and their families, taking emergency calls on phone provided by company, delegation of medication administration and oversight of medication administration program, and documentation in resident records as well as other miscellaneous required forms.
Primary Duties:
1. Participate in completing pre-admission assessments with a member of the location management team (LPN or other RN, Administrator, Director) on pre-admission assessments. 2. Complete all fall, pain, evacuation, and elopement assessments for each resident. 3. Complete all quarterly and monthly psychotropic med reviews. 4. Delegate all RN specific medication tasks that can be completed with RN oversight. 5. Manage all clinical staff at Anita's Gardens (LPNs). 6. Using compliance checklist, check each location monthly for issues of non-compliance (med cart audits, narcotic counts, missing med reports).
Secondary Duties:
1. Ensure medication administration, health monitoring and auditing of medication cart are done monthly. Participation in mediation administration is needed to ensure staff continue to follow regulations. 2. Ensure narcotic documentation is being done properly and narcotic counts are accurate. 3. Ensure physician orders, medications, and medication administration records (MAR) are all accurate and properly documented. 4. Assist in the provision of training and oversight of staff. 5. Communicate with residents, management, staff, physicians, other health care professionals and families regarding resident well-being such as; medication changes, care plan updates and changes in condition. 6. Prepare notes and vitals for Bluestone residents. 7. Continually monitor resident health and follow up with appropriate parties as needed. 8. Check in new medications coming in via pharmacy. 9. Destroy discontinued and missed medications in a proper manner. If narcotics need to be destroyed, 2 managerial personnel need to be present. 10. Document changes in condition in resident record and update care plan as needed. 11. Complete care plan in conjunction with management, resident, family and social worker every six months or when a change of condition occurs. 12. Actively participate in finding workable solutions to concerns and issues. 13. Participate in staff and resident meetings. 14. Help prepare resident notes for medical appointments as needed and review all new orders from appointments on return. 15. Provide and read Tuberculosis skin tests for new employees. 16. Order labs, x-rays, and other imaging as ordered by medical team and relay results to said team. 17. Provide on-going staff education of vital signs, glucose monitoring, emergency intervention, etc. 18. Assist in daily operations. 19. Perform admission assessments as a team and offer input on placement. 20. Provide input in employee reviews and disciplinary actions when appropriate. 21. Assist in creating and enforcing infection control policies and procedures. 22. Change catheters if necessary. 23. Report back to Regional Director with any concerns regarding operations.
Other Requirements:
1. Extended hours and irregular shifts may be required, especially in situations that cannot be avoided such as; severe weather, employee injury, etc. Up to 4 hours is expected, until someone can come in to relieve you. 2. Expected to frequently walk, stand, bend, lift, reach, communicate, listen, etc. 3. Occasionally stoop, kneel, crouch or crawl. 4. Ability to lift and/or move up to 35 pounds. 5. Follow DHS guidelines and Anita's Gardens policies and procedures. 6. To work at least three, eight hour shifts per week. 7. Cover on-call hours in evenings and weekends.
Qualifications:
1. Must have current RN license 2. Must have at least 2 years' experience working with the elderly population. 3. Experience training staff 4. Ability to exercise independent judgement and work independently. 5. Ability to operate various types of medical equipment as assigned. 6. Ability to maintain good relationships with individuals of varying social and cultural backgrounds. 7. Must be sensitive, compassionate and caring to the special needs of the residents. 8. Must have reliable transportation. 9. Must complete required training as needed to complete all CBRF certifications. 10. Must maintain an acceptable work attendance. 11.
