Job Description

Seeking a Registered Nurse (RN) Home Health for a part-time, ongoing role in Cascade, Wisconsin. 24 hours/week, day shifts, staff employment Performs scheduled patient visits, collaborates with interdisciplinary team, and conducts comprehensive assessments. Updates care plans, communicates with physicians, and monitors patient responses to treatments and medications. Partners with RN Case Manager for discharge planning and ensures continuity of care. Documents patient records per guidelines, follows policies, and performs patient positioning and transport duties. Provides age-appropriate care, demonstrating growth and development principles. Requires RN license, BLS certification, and a valid driver's license; prefer 1 year of nursing experience. Must have strong clinical, communication, organization, and problem-solving skills, and be able to lift, push, and perform physical tasks safely.
