•Bonus available to external/new hires only Responsibilities As a RN Case Manager You will: Assess/ monitor physical, emotional, and psychological needs of patients Create hospice care plans that align with the patient's wishes and goals Direct nursing care: administering medications, treatments, and interventions Provide pain and symptom management Educate and support the patient's family and caregivers Collaborate with interdisciplinary team Maintain accurate and timely documentation Participate in on-call rotation as required by local branch Qualifications Graduate of an accredited nursing school with current licensure to practice in state of operation One year nursing experience required , 3+ years preferred Experience in hospice or a similar setting preferred Knowledge of the hospice philosophy of care Commitment to clinical and documentation excellence Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order About Us At Allay Hospice, our primary ambition is to bring first-class care to the warmest and most serene environment possible: home.
