Responsibilities include:
Supervising care, leading patient assessments, and developing personalized care plans. Building meaningful relationships with patients and their families to understand their unique needs. Collaborating effectively with multidisciplinary teams to ensure coordinated, high-quality care. Supporting fellow staff with complex patient needs and inquiries. Celebrating patient milestones and progress.
Qualifications:
Valid RN license as per state regulations. CPR certification (ACLS preferred). CRRN certification preferred. Experience in a rehabilitation hospital setting is advantageous.
