Job Description

POSITION SUMMARY
The Admissions Nurse is responsible for execution of the hospice admission process in compliance with federal and state regulations. The Admissions Nurse will advocate for the patient and family to meet those stated needs that hospice can cover and/or refer them to other resources as appropriate.
ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING
Demonstrate competency in meeting with potential patients and families in assessment of their needs. Maintain confidentiality and safety of patient/client clinical records. Complete assigned admissions in an effective and timely manner. Communicate effectively with physician(s), referral sources, patient, family, and hospice Interdisciplinary Group (IDG) regarding the new admission of a patient. Responsible for communicating and documenting the clinical assessment, identified needs, and other pertinent information. Record information in the medical record and pertinent medical history and assist in obtaining medical records that may be relevant to determining hospice eligibility. Assist in ordering emergency kit medications, as well as any medications that are needed immediately for new admissions (per physician order) and facilitate processing of the medication order(s) with the pharmacy provider. Assist with orders and appropriate education for DME as needed. Communicate effectively with operational, clinical, and sales leadership regarding status of pending referrals. Other appropriate tasks and special projects as assigned. QUALIFICATIONS & REQUIREMENTS FOR THE POSITION
Current RN licensure in state of practice. Minimum of two (2) years of experience working in hospice, home health, or other healthcare setting. Current CPR certification required. Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Limited (< 30%) of daily assignments may require travel to client/resident/patient locations or other work sites, via car or public transportation.
