Make a Difference Where It Matters Most! Holistic Home and Hospice is looking for a compassionate, skilled PRN/Part-Time RN Case Manager to join our hospice team. As a Hospice RN Case Manager, you'll have the opportunity to make a meaningful difference in the lives of patients and families during one of the most important times in their journey. What You'll Do Conduct initial and ongoing comprehensive assessments of hospice patients. Obtain medical histories and assess the patient's current physical and emotional condition. Perform physical assessments, including vital signs, mental status, appetite, dietary needs, and overall condition. Evaluate the patient, family, and home environment to identify care needs, available support, and appropriate services. Provide professional nursing care using the nursing process. Provide nursing services, treatments, and preventative procedures requiring specialized nursing skill and physician orders. Observe and report changes in the patient's condition, including responses to treatments and medications, to the appropriate physician. Educate patients and families regarding the plan of care, the dying process, symptom management, and healthcare decisions. Provide guidance and support to families as they navigate end-of-life care. Provide dietary guidance while taking into consideration the patient's health status and nutritional needs at the end of life. Perform death visits in accordance with agency policy. Participate in patient discharge planning when appropriate. Case Management & Care Coordination Coordinate hospice services with patients, families, physicians, and members of the Interdisciplinary Group (IDG) to ensure patient needs are met. Develop and implement the initial plan of care and collaborate with the IDG to maintain a comprehensive, individualized plan of care. Keep patient plans of care current and make revisions as patient needs change. Consult with the attending physician and Hospice Medical Director regarding changes in patient condition, orders, or plan of care. Coordinate services with contracted providers and other agencies when needed to promote continuity of care. Understand different levels of hospice care and when each may be appropriate. Explain nursing and other agency services to patients and families as part of care planning. Evaluate the effectiveness of hospice and nursing services for the patient and family on an ongoing basis. Participate in IDG meetings and case conferences. Submit weekly patient visit schedules and complete, accurate, timely clinical documentation. Team Leadership & Collaboration Supervise and evaluate Hospice Aide care at least every 14 days and as needed. Develop the plan of care for the Hospice Aide. Provide guidance to LPNs and complete required monthly supervision. Communicate with supervisors regarding patient needs, concerns, and potential solutions. Work collaboratively with physicians, nurses, aides, contracted providers, and other members of the care team. Step in to support other nursing team members when requested by a supervisor. Quality & Professional Development Participate in staff development meetings. Maintain current knowledge of medications and drug therapy. Stay current with hospice nursing practices through in-services, continuing education, workshops, conferences, professional organizations, research, and reading. Maintain compliance with federal, state, Medicare, Medicaid, and accreditation requirements. What We're Looking For Graduate of an accredited School of Nursing. Current Registered Nurse (RN) license in the State of Wisconsin . At least 1 year of RN experience ; hospice experience is preferred. Strong clinical assessment and problem-solving skills. Excellent organization, time-management, and case-management skills. Strong verbal and written communication skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Valid driver's license and reliable, insured automobile in good working condition. Willingness and ability to travel throughout assigned service areas. Ability to participate in on-call coverage approximately 4-6 times per month, including holidays as required by agency policy and patient needs. What the Role Requires Because hospice care takes place where patients call home, this position requires the ability to travel to patient residences and work in a variety of home environments. 100% willingness to travel is a must!

The role may involve:

Walking, sitting, standing, stooping, and bending. Physical assessment and demonstration of patient care. Assisting with patient positioning and turning. Minimal to maximum lifting of patients as required by the patient's care needs. Accessing homes that may not be routinely wheelchair accessible.
