Key Responsibilities:

Complete comprehensive home health risk assessments for Medicare Advantage members Review and evaluate prior/current medical history, ongoing medical conditions, and medication regimens Provide patient education regarding diagnoses and prescribed medications Recognize gaps in care and recommend appropriate next steps Administrative follow-up; no prescribing or treatment planning required

Qualifications:

Active Nurse Practitioner license in Wisconsin (WI) Board certification from AANP or ANCC required Current BLS certification (CPR) Graduation with a Master of Science in Nursing (MSN) from an accredited program Six months of recent Nurse Practitioner experience (preferred, not required)
