Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage plan members Review and evaluate medical history, current health conditions, and medications Educate patients on managing their medical conditions and understanding medications Identify care gaps and make recommendations to optimize patient health Visits may include annual assessments and post-discharge evaluations No responsibilities for creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Wisconsin Current BLS (CPR) certification Board Certification through AANP or ANCC required At least 6 months of current experience as a nurse practitioner preferred MSN degree from an accredited nursing school This position compensates $80-$90 per evaluation, with additional mileage reimbursement and administrative time included.
